OAFC Memorial Service Information

Annually, at the OAFC Conference, a memorial service is held to honour the chief officers who have passed away since our last conference.

A Chief Officer from the home department will be given the opportunity to lay the helmet at the ceremony to honour the Chief Officers that have passed since we last met in person. If no one is selected from the home department, the President or designate will appoint a Chief Officer to act on behalf the home fire department.

This form can be used to submit name(s) for the 2024 memorial presentation. The fire department contact named on this form will be notified prior to the event with further details.

Any names submitted after March 29, 2024, will be memorialized at the 2025 memorial.
