Solar Electricity Safety Training for Fire Departments Course Request Form

Process/Instructions:

Once you have completed and submitted the online course request form, an OAFC coordinator will follow up with you in regards to details for your course. 
 
If you do not have an internal instructor, ensure you note that on the form below.  The OAFC coordinator will secure one for you.  Once your instructor has been confirmed, we will be in contact with you to facilitate discussions regarding training room setup/etc.
 
Departments/hosts must provide the following for the course:
 
1.    Projector, screen and computer.
2.    Flip chart and markers
3.    Room for up to 40 attendees
 
Departments are responsible for submitting a registration list for your course, not later than ten (10) business days prior to the course.   This is a requirement in order to prepare and mail course materials.
 
Once all information is received, course materials will be prepared and sent to your department.  The course has an evaluation process.  We ask that all participants complete the evaluations.  The OAFC collects the data for all courses to ensure quality control, consistency and for further course developments.
 

Course Fees:

Internal Instructor:  Select this option if you have someone in your department that is an OAFC qualified solar instructor, and you want to do the training within your own department.
 
Cost:  $12.76 per person, plus a $13.00 fee for one Electrical Safety Handbook for Emergency Responder handbook.  This book is referenced in the course materials, and must be on hand should students wish to refer to it.   The Solar Electricity Safety Handbook for Firefighers is included in the $12.76 fee for each student.  If you wish for each student to receive a copy of the Electrical Safety Handbook for Emergency Responders, the cost is $25.76 per student.
 
External Instructor:  Select this option if you do not have anyone in your department that is an OAFC qualified solar instructor.  
 
Cost:  $13.32 per person, plus instructor expenses (mileage & $250 instructor fee) and a $13.00 fee for one Electrical Safety Handbook for Emergency Responder handbook.  This book is referenced in the course materials, and must be on hand should students wish to refer to it.  The Solar Electricity Safety Handbook for Firefighers is included in the $13.32 fee for each student.  If you wish for each student to receive a copy of the Electrical Safety Handbook for Emergency Responders, the cost is $26.32 per student.
 
The OAFC will invoice the department for the course cost plus instructor expenses (for external courses only).  The OAFC coordinator will make every effort to secure instructors that are closest to your department to keep costs low.

Terms and Conditions
The information provided by me on this course request form is as accurate and as up to date as possible. I acknowledge that I have the applicable permissions to request and secure such a course, on behalf of my fire department/zone/mutual aid or applicable fire service group. I acknowledge and agree that this training is for the fire service. I agree to pay the cost of the program and pay all instructor fees associated with this course, if applicable. Instructor fees are based on mileage and accommodations (if necessary). The OAFC coordinator will attempt to secure trainers closest to your location as possible. There are no guarantees. Once course is confirmed, I agree to provide the OAFC coordinator with participant information no later than ten (10) business days before the start date of my course. I understand that there is an evaluation process to this program, and although not mandatory, will help further develop and maintain the course, and I will promote the evaluation process to the participants in my course. Courses cancelled outside of ten (10) business days, will not be charged a penalty. Courses cancelled with ten (10) business days, will be charged at minimum, a $50 cancellation fee, at maximum, $150, unless provisions/rescheduling can be made, without penalty to the instructor(s) or the OAFC at a mutually beneficial date/time. The cancellation fee is set by the OAFC based on costs incurred that will not be recovered (can include material costs/accommodation for instructor etc) This is at the sole discretion of the OAFC
